جو بہت تیز اور مضبوط ہوتے ہیں۔
اس کے پتے ثلاثی یعنی تین تین ہوتے ہیں۔
دو پتے بالمقابل ہوتے ہیں۔

موسم گرما سے قبل اس کے پرانتے پتے جھڑ جاتے ہیں۔
اور چیت بیساکھ میں نئے پتے نکل آتے ہیں ۔
جن کا رنگ سُرخ ہوتا ہے۔
لیکن بعد میں سبز رنگ کے ہوجاتے ہیں۔
موسم گرما کے آتے ہی پھل پکنے شروع ہو جاتے ہیں۔
اس وقت درخت کے تمام پتے جھڑ جاتے ہیں۔
اور درخت پر صرف پھل ہی رہ جاتے ہیں۔
پھل جسامت میں نارنج کے برابر ہوتا ہے۔
جس کا وزن آدھ پائو سے لے کر نصف سیر تک ہوتا ہے۔
اس کا چھلکا دبیز،  سخت اور چکنا ہوتا ہے ۔
اس کے اندر سخت گودا بھرا ہوتا ہے۔
پکنے پر گودا نرم ہوجاتا ہے۔
اور اس میں مٹھاس آجاتی ہے۔

ہندو اس درخت کو بڑی عزت کی نگاہ سے دیکھتے ہیں۔
اور اس کو ضائع کرنا گنا ہ سمجھتے ہیں۔
اور اس کے پتوں کو شیو جی کی مورتی پر چڑھاتے ہیں۔

میرٹھ کا کا غذی بیل بہترین سمجھا جاتا ہے۔
اس میں بیج بہت کم ہوتے ہیں۔
موسم سرما کے خاتمہ پر پھول چھوٹے چھوٹے سفید رنگ کے آتے ہیں۔
پھولوں میں شہد کی مانند خوشبو آتی ہے۔

اس میں ٹے نین کے سوا
کوئی دوسراجزو موثرہ تا حال دریافت نہیں ہوا ۔

بیل گری کے فوائد

حصص مستعملہ :۔
جڑ پوست پتے اور پھل پھل کا گودا دوائی استعمال کے لیے خام پھلوں کا خشک گودا
ہی کام میں لایا جاتا ہے۔

مربا کے لیے آدھے پکے پھولوں کا گودا استعمال کیا جاتا ہے۔

پختہ پھلوں کے گودا کا شربت بنایا جاتا ہے۔

دشمول وغیرہ جو شاندوں کے لیے جڑ یا درکت کا پوست برتا جاتا ہے۔

رنگ:۔ پھل شروع میں سبز رنگ کے ہوتے ہیں اور ان پر قدرے نیلے رنگ کی جھلک ہوتی ہے پکنے پر زرد ہوجاتے ہیں۔

قابض ہونے کی وجہ سے پرانے دستوں کو بند کرتا ہے۔

پرانی پیچس میں فائدہ کرتا ہے۔

آنتوں کو صاف کرتا ہے۔

آئوں اور خون کو بند کرتا ہے۔

بچوں کو اسہال و پیچس میں بہت فائدہ دیتا ہے۔

According to International guidelines, unless a patient’s platelet count is below 10,000, and there is spontaneous, active bleeding, no platelet transfusion is required. The outbreak of dengue in the Capital has given rise to extreme panic. Hospital beds are full and families are seen running around in search of platelets for transfusion. However what most people do not realize is that the first line of treatment for dengue is not platelet transfusion. It, in fact, does more harm than good if used in a patient whose counts are over 10,000.

The primary cause of death in patients suffering from dengue is capillary leakage, which causes blood deficiency in the intravascular compartment, leading to multi-organ failure. At the first instance of plasma leakage from the intravascular compartment to the extravascular compartment, fluid replacement amounting to 20 ml per kg body weight per hour must be administered. This must be continued till the difference between the upper and lower blood pressure is over 40 mmHg, or the patient passes adequate urine. This is all that is required to treat the patient. Giving unnecessary platelet transfusion can make the patient more unwell.

Speaking about the issue, Padma Shri Awardee, Dr KK Aggarwal, President HCFI & Honorary Secretary General IMA said, “While treating dengue patients, physicians should remember the ‘Formula of 20' i.e. rise in pulse by more than 20; fall of BP by more than 20; difference between lower and upper BP of less than 20 and presence of more than 20 hemorrhagic spots on the arm after a tourniquet test suggest a high-risk situation and the person needs immediate medical attention.”

Dengue fever is a painful mosquito-borne disease. It is caused by any one of four types of dengue virus, which is transmitted by the bite of an infected female Aedes aegypti mosquito. Common symptoms of dengue include high fever, runny nose, a mild skin rash, cough, and pain behind the eyes and in the joints. However, some people may develop a red and white patchy skin rash followed by loss of appetite, nausea, vomiting, etc. Patients suffering from dengue should seek medical advice, rest and drink plenty of fluids. Paracetamol can be taken to bring down fever and reduce joint pains. However, aspirin or ibuprofen should not be taken since they can increase the risk of bleeding.

The risk of complications is in less than 1% of dengue cases and, if warning signals are known to the public, all deaths from dengue can be avoided.

Answer- Dangerous injury is part of Grievous injury as per 8th clause in Section 320 IPC
“Any hurt which endangers life, or causes the victim to be in severe

bodily pain for 20 days, or unable to do ordinary pursuits of life for 20 days” is clause 8 of Grievous hurt

If injury is dangerous to life, it is mandatory to mention it also in MLC as it helps police in slapping Section 307 IPC on accused.

Section 307 in The Indian Penal Code

307. Attempt to murder.—Whoever does any act with such intention or knowledge, and under such circumstances that, if he by that act caused death, he would be guilty of murder, shall be punished with imprisonment of either description for a term which may extend to ten years, and shall also be liable to fine; and if hurt is caused to any person by such act, the offender shall be liable either to 1[imprisonment for life], or to such punishment as is hereinbefore mentioned. Attempts by life convicts.—2[When any person offending under this section is under sentence of 1[imprisonment for life], he may, if hurt is caused, be punished with death.]

INTRODUCTION
Gilo  (Tinospora cordifolia)

Tinospora cordifolia is a large, glabrous, deciduous  climbing shrub belonging to the family Menispermaceae.

It is distributed throughout tropical Indian subcontinent and China, ascending to  an altitude  of 300 m.

The plant is commonly known as  Guduchi,  Giloy  or  Amritha, which are Hindu mythological terms that refer to the heavenly elixir that have  saved celestial beings from  old age and kept them  eternally young.

The same  things describe in  Unani  literature as “Ikseere badan /  Aab-e-Hyat” meaning water of  life (Elixir of  Life),  i.e.  Ikseere  badan Advia are that drugs which promote healthy and delay aging or  heavenly elixir having saved celestial beings from old age and  kept them  eternally young

Gilo is  widely  used in  veterinary folk medicine and  Ayurvedic  system  of medicine for its general  tonic, anti-periodic, anti-spasmodic, anti-inflammatory, anti-arthritic, anti-allergic and anti-diabetic properties.

In  Ayurveda, it is designated as  Rasayana  drug recommended to enhance general body resistance, promote longevity and as anti-stress and adaptogen

The whole plant is used in Ayurvedic “Rasayanas” to improve the immune system  and the body resistance against infections  and root is known for its anti-stress, anti-leprotic and anti-malarial activities.

The fact that it is called “Amrita” signifies  its use for revitalization  and its  importance in  Ayurveda  and in  Unani  as  Ikseere Badan.

Gilo has been reported to be active against throat cancer in man and it has been reported  to be non-toxic in acute toxicity studies  in vivo, with almost no side effects.

 It is a climbing deciduous shrub. It is found throughout tropical part of India, also found in China,  Bangladesh, Myanmar, Srilanka, South Asia, Indonesia, Phillipians and Thailand.

T.cordifolia  prefers wide range of soil, acid to alkaline and it needs moderate level of soil moisture, ascending to an altitude of  1000 feet in worldwide.

Phytochemical Constituents:

There are various chemical constituents  have been discovered from  different parts of the Gilo plant which  have different pharmacological action have been proved.

Anti-leprotic, Antidote,

Helpful in Leucorrhoea,
Laxative, Anti-Filaria,  Effective in splenomegaly and Seminoviscous.

Gilo uses in  Unani  Medicine

Because of various  medicinal  properties Gilo  has bee advocated for following disease in  unani system  of medicine;

Fever, Diabetes, Jaundice,  dry and productive cough, Loss of appetite, Loss of Libido, Syncope, Haemorrhoid, Nausea, Cancer, Epilepsy, Melancholia, Mania, Leprosy, Gout, Leucorrhoea, Constipation, Filaria,  Splenomegaly, Oligospermia.

Gilo is one of the chief  ingredient of all the  unani  product which are being used to manage diabetes;

Murakabaat which is  having Gilo as chief ingredient are

 SCIENTIFIC STUDIES

 Antioxidant activity:

Aqueous extract of Gilo compact  the toxic side effects of cyclophosphamide in mice as revealed by the entirety white blood cell count bone marrow cellularity and esterase positive cells

The leaf extract powder showed higher retention of antioxidant activity than stem extract powder.

Methanolic, ethanolic and aqueous extracts of  Gilo showed significant antioxidant potential compared to other solvents and also  possess metal chelation and reducing power activity.

In the DPPH radical scavenging activity, methanolic  extract (98.13%) showed high antioxidant

potency, ethanolic extract (90.34%) was a potent scavenger of superoxide radical. At the same time, both methanolic (97.08%) and ethanolic extracts (95.21%) inhibited hydroxyl radical along with other extracts.

The metal chelation in methanolic (60.62%), ethanolic (57.62%), aqueous extracts (40.89%) and reducing of  ferrous ions was significant  found increasing in methanolic, ethanolic and aqueous extracts.

In another study it was found that aqueous and ethanolic extracts  of  T. Cordifolia  were subjected to  in vitro  antioxidant  activity screening models such as DPPH, ABTS, nitric oxide and  superoxide radical scavenging  activity, inhibition of lipid peroxidation, reduction of ferric ions and total antioxidant capacity.

Immunomodulatory activity:

Gilo have active compounds like

11-hydroxy mustakone, N-methyl-2-pyrolidone,

nformylannonain, cordiflosid,

tinocordiside and syringin have  potential immunomodulatory effect.

Treatment with T. cordifolia  extract (10 mg/kg bw for 15 days) in intoxicated  mice ameliorated the immunosuppressive effect.

The immunomodulatory activity of Gilo in different extracts, fractions and isolated compounds in relation  to phagocytosis  and reactive oxygen species production in human neutrophil cells have been investigated using the polymorphonuclear neutrophil phagocytic function  studies, nitroblue tetrazolium, nitrous oxide; and chemiluminescence assay.

There are seven active immunomodulatory compounds belonging to different classes have  been isolated from  T.cordifolia  and characterised indicating that the immunomodulatory activity.

The dry stem  crude extract (DSCE) of  Tinospora cordifolia contained a polyclonal B cell mitogen, G1-4A. DSCE as well as G1-4A also enhanced immune response in mice.

In order to  explore the possibility  of using G1-4A/PPI (partially purified immunomodulator) to modulate radiation induced immunosuppression,  the antioxidant effect of PPI  from  this plant was examined against reactive oxygen and nitrogen species (ROS/RNS), generated by  photosensitization/peroxynitrite.

Hepatoprotective activity:

Leaf extracts of  T. cordifolia  exhibited a hepatoprotective effect against carbontetrachloride induced hepatotoxicity in rats.

It is reported that stem  and leaf extract of T.cordifolia has hepatoprotective effect in  lead induced toxicity  in Swiss  albino male mice.

When aqueous extract of stem  gilo provide in  carbon tetrachloride intoxicated rats it was found to be reduced elevated serum  liver enzyme  level to  near control levels.

Some  important alkaloids  identified in the stem  and roots from  Tinospora cordifolia  which are tinosporine and tinosponone, reported as
hepatoprotective agents  and have exhibited  in vitro  inactivating property against Hepatitis-B and E.

The  T. cordifolia  was reported as hepatoprotective action  in the experiment in which goats treated with  T. Cordifolia.

 It  have shown significant clinical and hemato-biochemical improvement in carbon tetrachloride induced hepatotoxicity.

Anti-tumour activity:

The anticancer activity of dichloromethane extract of  guduchi   in the  mice transplanted with Ehrlich  Ascites  carcinoma  (EAC) was investigated, The administration of various doses of guduchi  extract  caused a dose dependent retardation in  the  tumour development, as is evident by the inhibition in body weight gain and increase in  the life span (MST and AST). This  indicates to the effectiveness of  guduchi  extract in restricting tumour  cell multiplication and growth.

Tinospora cordifolia extract may have caused  DNA damage in the EAC cells leading to the cytotoxic effect.

Tinospora cordifolia extract contains berberine that has been found to be a topoisomerase-II inhibitor.

Other topoisomerase II  inhibitors like etoposide and teniposide have been found to exert antineoplastic activity.

In Anticarcinogenic studies, C57BL mice which received  Tinospora cordifolia  extract at the dose of 750 mg/kg for 30 days showed increase in life span of animals  and tumour size was significantly reduced in  Tinospora cordifolia  treated mice as compared to  Control.

Hypoglycaemic activity:

Tinospora cordifolia  (TC) stem  extracts (both aqueous and  alcoholic) in different dosages (200 and 400 mg/kg b.w.) in streptozotocin diabetic albino rats, significantly decrease the blood sugar.

Tinospora cordifolia  is an effective antihyperglycemic drug that can be used in the treatment of DM.

Since TC increased the activity of glycogen synthase  in the liver, it may increase the storage of glucose in hepatocytes.

 It also decreased the activity of phosphorylase in the liver; thereby it may prevent the release of  glucose into the blood.

It was found that aqueous and alcoholic extracts of  Tinospora cordifolia  could ameliorate the metabolic derangements in lipid metabolism  caused by STZ induced diabetes in rats.

Oral administration of an aqueous  T. cordifolia  root extract (TCREt) to alloxan diabetic rats caused a significant reduction in blood glucose and brain  lipids.It has hypoglycaemic and hypolipidaemic effect.

Tinospora cordifolia  prevents the hyperalgesia in experimental diabetic neuropathy.

The results showed that the root  extract of Tinospora cordifolia  is pancreatoprotective and is hypoglycaemic in nature.

The oral administration of various extracts (hexane, ethyl acetate and methanol) of Tinospora cordifolia stem (TCS)  were found to have potent anti-diabetic activity that reduces blood sugar level in streptozotocin-(STZ) induced diabetic rats.

Anti-HIV  Activity:

The leaves of  T. cardifolia  extracts shows anti-HIV-I activity.

A clinical evaluation of the Gilo extracts for immunostimulant action and reduced the esonophil count, increase haemoglobin percentage and stimulate the secretion of B-lymphocytes against the symptoms  of HIV.

Antimicrobial activity:

Antibacterial activity of  aqueous, ethanol and  chloroform extracts of  leaves and  stem of Tinospora cordifolia  were tested on clinical isolates of urinary pathogens.

Ethanol extract of leaf showed greater inhibitory  action than  other tested  extracts.

The crude extracts of  T. cordifolia stem  showed  activity against bacteria and fungi.

  Anti-ulcer  activity:

An ethanolic extract of the roots of  Tinospara cordifalia  in combination with  Centella asiatica  afforded significant protective action  against restraint stress induced ulcer formation.

 It  was  found  that  T.  Cordifolia root extract  has also shown some  promising speed in healing the surgical diabetic  foot ulcers.

Anti-pyretic activity:

The water-soluble fraction  of  95% ethanolic extract of  T. cordifolia  plant has shown significant antipyretic  activity.

another experimental study, antipyretic effects have been reported in  the hexane and chloroform-soluble portions of  T. cordifolia  stems.

Various studies show remarkable anti-infective and antipyretic properties of  T. Cordifoli.

Anti-hyperlipidemic activity:

Administration of the extract of  T.  cordifolia  roots  for 6 weeks resulted in a significant reduction in serum  and tissue cholesterol, phospholipids and free fatty acids  in alloxan diabetic rats.

Tinospora cordifolia  root extract significantly decrease the level of cholesterol, triglycerides, LDL, VLDL, blood glucose and increase the level of HDL cholesterol.

CONCLUSION

It is well known fact that the various medicinal plants describe in  Unani  medicine play a vital role against range of diseases.

Herbs as a whole or their extracts have  significant medicinal values.

Our review result shows that Gilo could  prevent from  Fever, Ulcer, Diabetes and Cancer and many more ailments.

A variety of  unani  products have been reported to possess that activity.

Hence the review study  is concluded that the Gilo  possesses antiulcer, antipyretic, anti-diabetic,
anti-cancerous etc.,

Activities and it has been proved by different animal models also; which give many links to develop the future trials.

Query- In private nursing homes, lot of BAMS doctors work as Medical Officers. How safe it is to continue this in court of law?   If untoward incident happens, can a BAMS doctor share the responsibility via a vis MBBS doctor?

Answer- It is sad reality that a lot of BAMS doctors work as resident in many private nursing homes or hospital. It is grossly illegal as they are not trained in modern practice.
In case of negligence, doctor would be charged of taking help from unqualified staff and entire blame would go on primary physician. BAMS doctors are appointed instead of MBBS doctors as they charge one third salaries. Later they practice full time our system of medicine
