A high-precision and excellently coordinated effort between two cities led to a successful heart transplant in Mumbai today. The transplant was coordinated between Mumbai and Pune for a 22 year old recipient, suffering from Cardiomyopathy, admitted at Fortis Hospital, Mulund. The donor heart came from Jehangir Hospital, Pune, pledged by a 42year old donor.
The journey of the heart, retrieved by Dr Sanjeev Jadhav, Consultant CVTS Surgeon, Fortis Hospital, Mulund, began from Pune, with due consent being taken from the donors family who agreed to donate and save a life. With the “Green Corridors” enabled by the Pune and Mumbai Police and Airport Authorities, the heart reached, Fortis Hospital, Mulund in 60mins flat after being harvested! The usually busy 19 km route from Mumbai Airport was cleared of traffic by the authorities so that the preserved heart could reach the Operation Theatre at Fortis Hospital, Mulund in a record time of 18 minutes straight into the waiting hands of Dr Anvay Mulay, Head of Cardiac Surgery and his team.
Mr Sukhmeet Sandhu, Regional Director, Fortis Healthcare (Region-West & East), said, “This is a momentous occasion for Fortis and the city of Mumbai. I congratulate the medical teams and particularly the traffic authorities both at Pune & Mumbai. I’m sure this is the beginning that places our city firmly on the map as far as heart transplants are concerned”.
Hon. Milind Bharambe, Joint Commissioner, Traffic, Mumbai, said, “Meticulous planning and timely execution made this feat possible. Advancing from the Green Corridor in Pune, the heart was airlifted from Pune Airport to Mumbai, moving further from Santacruz Airport(Gate No. 8) to Military Road to Santacruz Chembur Link Road to Chedda Nagar to Eastern Express Highway to Airoli Junction and finally to Fortis Hospital, Mulund. Saving a valuable life made it well worth the effort and we are grateful that the citizens of Mumbai co-operated willingly in this humane endeavour”.
Dr S Narayani, Zonal Director, Fortis Hospital, Mulund said, “It is indeed a proud moment for Mumbai, and for all of us at Fortis Mulund in what is the cities 1st heart transplant.. This move strengthens our resolve in making Mumbai a preferred healthcare destination for organ transplantation, particularly heart transplants. This is not just a case study of Clinical Excellence for the hospital but also a testament to the massive coordination efforts between the Traffic authorities, medical teams and the community in both the cities.”
Dr Anvay Mulay, Chief of Cardiac Surgery, Fortis Hospital, Mulund, said, “This is the beginning of a robust Heart Transplant Programme in Mumbai and will help save many lives”.
The request to enable the 'Green Corridor' was received by the Pune traffic police, the Airport Authority of India and the Mumbai Traffic Police around 10am today. The authorities were quick to act and the two cities were ready with the arrangements for the Green Corridor within 20 minutes from the time of initial request to aid. From clearing the traffic on the roads, to stalling red light signals, flight coordination, was done smoothly and in record time. This proved to be a well-orchestrated symphony between the two cities and its local administrations.
This feat has been possible with great impetus from the media who has relentlessly followed the cause of organ donation, Dr Mulay’s Clinical Team and the Traffic Authorities who redefined transportation time.

Habitually using handphones b4 bedtime in the dark can lead to serious problems.
Recently, an increasing number of 30-40 year-old patients are seeking medical attention due to using handphones in the dark.
Professor Li Li, director of the Hospital of Ophthalmology said: direct glare of >30 minutes would cause irreversible eye macular degeneration that leads to rapid deterioration of vision.
Contracting maculopathy (eye cancer) means waiting for loss of sight because modern medicine cannot treat, let alone cure it.
The bright phone in the dark with close-up view & high-energy directed at the eyes will damage the macula of the eye.
Professor Li Li said that symptoms of macular degeneration is mostly experienced by old people, but lately patients are getting younger. Patients who are 30-40 years of age increased by 3 %, most are frequent users of handphones.
However, viewing the handphone in the dark not only causes macular degeneration. It initially causes dry eyes, cataracts and eventually loss of sight.
Early lesions of the eye have to be treated with laser or injection of steroids.
254 Hospital eye specialist Professor Li Li suggested that the most important thing is to get rid of the bad habit of using handphones in the dark. Because habitual use of handphones in thr dark b4 bedtime may cause a lifetime of harm.
Friends : In order to look after ourselves and our families, remember not to turn off the lights if viewing the handphone. Inform handphone users urgently that viewing handphones in the dark can easily lead to loss of sight!

The journey of the heart, retrieved by Dr Sanjeev Jadhav, Consultant CVTS Surgeon, Fortis Hospital, Mulund, began from Pune, with due consent being taken from the donors family who agreed to donate and save a life. With the “Green Corridors” enabled by the Pune and Mumbai Police and Airport Authorities, the heart reached, Fortis Hospital, Mulund in 60mins flat after being harvested! The usually busy 19 km route from Mumbai Airport was cleared of traffic by the authorities so that the preserved heart could reach the Operation Theatre at Fortis Hospital, Mulund in a record time of 18 minutes straight into the waiting hands of Dr Anvay Mulay, Head of Cardiac Surgery and his team.
Mr Sukhmeet Sandhu, Regional Director, Fortis Healthcare (Region-West & East), said, “This is a momentous occasion for Fortis and the city of Mumbai. I congratulate the medical teams and particularly the traffic authorities both at Pune & Mumbai. I’m sure this is the beginning that places our city firmly on the map as far as heart transplants are concerned”.
Hon. Milind Bharambe, Joint Commissioner, Traffic, Mumbai, said, “Meticulous planning and timely execution made this feat possible. Advancing from the Green Corridor in Pune, the heart was airlifted from Pune Airport to Mumbai, moving further from Santacruz Airport(Gate No. 8) to Military Road to Santacruz Chembur Link Road to Chedda Nagar to Eastern Express Highway to Airoli Junction and finally to Fortis Hospital, Mulund. Saving a valuable life made it well worth the effort and we are grateful that the citizens of Mumbai co-operated willingly in this humane endeavour”.
Dr S Narayani, Zonal Director, Fortis Hospital, Mulund said, “It is indeed a proud moment for Mumbai, and for all of us at Fortis Mulund in what is the cities 1st heart transplant.. This move strengthens our resolve in making Mumbai a preferred healthcare destination for organ transplantation, particularly heart transplants. This is not just a case study of Clinical Excellence for the hospital but also a testament to the massive coordination efforts between the Traffic authorities, medical teams and the community in both the cities.”
Dr Anvay Mulay, Chief of Cardiac Surgery, Fortis Hospital, Mulund, said, “This is the beginning of a robust Heart Transplant Programme in Mumbai and will help save many lives”.
The request to enable the 'Green Corridor' was received by the Pune traffic police, the Airport Authority of India and the Mumbai Traffic Police around 10am today. The authorities were quick to act and the two cities were ready with the arrangements for the Green Corridor within 20 minutes from the time of initial request to aid. From clearing the traffic on the roads, to stalling red light signals, flight coordination, was done smoothly and in record time. This proved to be a well-orchestrated symphony between the two cities and its local administrations.
This feat has been possible with great impetus from the media who has relentlessly followed the cause of organ donation, Dr Mulay’s Clinical Team and the Traffic Authorities who redefined transportation time.

Habitually using handphones b4 bedtime in the dark can lead to serious problems.
Recently, an increasing number of 30-40 year-old patients are seeking medical attention due to using handphones in the dark.
Professor Li Li, director of the Hospital of Ophthalmology said: direct glare of >30 minutes would cause irreversible eye macular degeneration that leads to rapid deterioration of vision.
Contracting maculopathy (eye cancer) means waiting for loss of sight because modern medicine cannot treat, let alone cure it.
The bright phone in the dark with close-up view & high-energy directed at the eyes will damage the macula of the eye.
Professor Li Li said that symptoms of macular degeneration is mostly experienced by old people, but lately patients are getting younger. Patients who are 30-40 years of age increased by 3 %, most are frequent users of handphones.
However, viewing the handphone in the dark not only causes macular degeneration. It initially causes dry eyes, cataracts and eventually loss of sight.
Early lesions of the eye have to be treated with laser or injection of steroids.
254 Hospital eye specialist Professor Li Li suggested that the most important thing is to get rid of the bad habit of using handphones in the dark. Because habitual use of handphones in thr dark b4 bedtime may cause a lifetime of harm.
Friends : In order to look after ourselves and our families, remember not to turn off the lights if viewing the handphone. Inform handphone users urgently that viewing handphones in the dark can easily lead to loss of sight!

If blood is coughed out & is mixed with sputum.
If blood is bright red, frothy with sputum.
Litmus paper = alkaline reaction.
If h/o epigastric ie, ulcer like pain. If h/o alcoholism & cirrhosis.
If blood is vomited out and is mixed with food particles.
If blood is coffee ground but it may be fresh red if variceal bleeding.
Litmus test = acid reaction.
Step-2 : start 18 no. I.V line
Start R-L or D N S, using 18 no.scalp vein needle and let the I.V run in very fast.
If lot of blood is lost, start hemacele/dextran (plasma expanders)
Make arrangements to shift the patient to a hospital with blood transfusion facilities.If hemetemisis,
Ryle’s tube and cold water stomach wash.
Inj.ranitidine 1-2 amps.I.VIf hemoptysis,
Inj.cefazolin 500 mg I.V 6 hrly.
x-ray chest and if hilar shadow, then bronchoscopy.
Anti-T B treatment, if tuberculosis.
Step-3 : measures to stop bleeding,
Inj.calcium gluconate 10 ml slow I.V stat.
Inj.dicynene 2 ml I.V stat. Or 4 hrly.(ethamsylate) or stryptohrome 2cc I M stat.(adrenochrome)
Inj.morphin 15 mg I M stat.
Inj.decadron 2 cc I V stat
SCORPION BITE (bichhu)
Patient present with very sever pain & sweating.
Inj. pethidine 100mg I.M/I.V stat. Or inj. fortiwin /norphin/morphin.
Inj. Local xylocain 2% : infiltrate at and around the site of bite (the site of bite is identified as pinpoint puncture spot, local sweating & edema ).
Tab. Prazopress 1 mg.stat. (prazocn = alpha bloker). Or ciplar 40 mg.stat. (propranolal ) In children, tab prazocin 1 mg x ½ -1/4 tab. Or tab.ciplor-10 mg. Tab. In most cases, where scorpion poison is mild, this treatment is sufficient. If there is systemic involvement,
Systemic involvement is indicated by – profuse sweating, ice cold extremities, priapism, hypersalivation, vomiting, hypertension, tachycardia & pulmonary edema.
Observe pulse rate, B P, & breathlessness, every 10 mins.
Tab. Prazopress 1 mg.stat ½ tab after 4 hrs, then ½ tab every 6 hrs till. Systemic symptoms & signs disappear, usually 24 to 48 hrs. or tab.ciplor 40 mg 1 stat.,1 after 4 hrs, then every 6 hrs.
Inj.fortwin 1cc I M/IV S O S if pain is severe.
I V. R L x if profuse sweating & dehydration.
If B P. > 150/100,
Tab. Depin 5 mg sublingual, to be repeated s o s after ½ -1 hrs (nifedipine).
If tachycardia >110/mins,
Inj.calmposed 2 cc I M or slow I.V
If breathlessness, fine fasal erepts i.e. pulmonary edema,
Propped up position.
Oxygen.
Inj. Lasix 2-4 amps I.V stat.
Inj. Efcorlin 100 mg I.V. stat
Inj. aminophyllin 10 ml + 25% dextrose 10ml x slow i.v or inj.deriphyllin 2 cc.i.m
In life threatening situations, with severe dyspnoea & frothing through mouth, give sodium nitroprusside drip till pulmonary edema is controlled.
Inj.pruside 50 mg in 5 ml x added to 500 ml D% x 15 drops/min x may be increased up to 40 drops /mins.
There is no specific antidote for scorpion poison. The magical effect of prazocin, which gives total protection against cardiac toxicity of scorpion bit was discovered by an Indian doctor, Dr. Bavaskar from mahad, raigad, district in maharashatra.
SNAKE BITE
First aid,
A reassurance is often the most important part of the panicked patient.
Tie a tourniquet above the site of bite on the limb, using a handkerchief or long piece of cloth, not too tight- just to occulude venous & lymphatic return.
Immobilize the limb with splint, because movements = more absorption.
If snake was poisonous, make the cruciate incision through the bite marks, and allow the blood to flow.Signs of poisoning.
Two puncture marks, 3/4-1 cm a part, should mark you suspicious of poisonous snakebite.
There are two type of poisons – paralytic type and hemolytic type
.
Paralytic type :
Stage- I : prosis.
Stage – II : difficulty in swallowing.
Stage – III: respiratory difficult & paralysis.
Hemolytic type:
Local swilling around the bite develops rapidly.
Blood collected in test tube does not clot.
If no signs of poison are seen,
Tab. Diazepam 1 stat (to relieve anxiety)
Tab. Disprin 1 stat (analgesic )
Inj. T.T. ½ cc I.M stat.
Reassure the patient.
Observe for 6 hrs, watch for the 4 signs of snake poison – local swelling, ptosis, dysphagia & respiratory difficulty. If signs of poison are seen,
Paralytic type:
Act immediate if ptosis or dysphagia is seen. If you do not have A.S.V, take the patient quickly to a hospital where A.S.V is available.remember that respiratory paralysis is imminent.
Inj.decadron 2 cc I.V stat
Inj. avil 1 amp. I.V stat
Inj. A.S.V. 1 to 4 vials, every 15 or 30 mins, till the signs start reversing (ASV-polyvalent). A.S.V. vial contains powder. Dissolve in 10 ml. distil water. Inj.½ cc I.V. as test dose. Wait for 2 mins. & Watch for urticaria. Then inject the full dose of a.s.v.
Inj. A.S.V.1 ml locally around the site of bite.

Recently, an increasing number of 30-40 year-old patients are seeking medical attention due to using handphones in the dark.
Professor Li Li, director of the Hospital of Ophthalmology said: direct glare of >30 minutes would cause irreversible eye macular degeneration that leads to rapid deterioration of vision.
Contracting maculopathy (eye cancer) means waiting for loss of sight because modern medicine cannot treat, let alone cure it.
The bright phone in the dark with close-up view & high-energy directed at the eyes will damage the macula of the eye.
Professor Li Li said that symptoms of macular degeneration is mostly experienced by old people, but lately patients are getting younger. Patients who are 30-40 years of age increased by 3 %, most are frequent users of handphones.
However, viewing the handphone in the dark not only causes macular degeneration. It initially causes dry eyes, cataracts and eventually loss of sight.
Early lesions of the eye have to be treated with laser or injection of steroids.

Start i.v drip of ringer’s lactate, with 18 no.needle, attach a 3-way, and push i.v. Fluids fast using a 20cc syring.
Push i.v fluids till B P rises above 100 mm
Inj.efcorlin 100 mg. X 1-2 vial i.v stat. Or inj.decadron or betnosol.
Inj.gentamycin 80 mg i.v 8 hrly.
Tab.lomotill 2 tds
Tab.spasmodan 1 tds
If B P does not raise after pushing 3-4 bottls,it is safer to refer the patient to a hospital.In a patient with diarrhoea & dehydration, never give D5%. Give electrolyte, DNS,RL, or NS.
The patient may be reqire 10 or more bottles of fluids, to be given within a short while.

Acute rise of blood pressure is potentially very dangerous, and can lead to hemiplegia due to cerebral haemorrhage
Immediate and complete bed rest.
Cap.depin 100 mg – punctured and aqueezed sublingually – (nifedipine) may be repeated after 15 mins. Or inj.reserpine 1 amp i.m stat or s o s
Inj.calmpose 2 cc i.m stat.
Simultaneously, start anti-hypertensive treatment, eg. Tab aten –50 1 b I d (atenolal = betablocker).or tab.envas 2.5 mg. 1 b.i.d (enalapril = ACE inhibitor )Subsequently :
Adjust the dose of anti – hypertensive drugs.
Salt free diet.
Instruct the patient o be regular in treatment & check up.
BREATHLESSNESS WITH WHEEZING
BREATHLESSNESS WITHOUT WHEEZINGacute exacerbation of bronchial asthma
inj.deriphyllin 2cc i.m stat
asthalin inhaler 1-2 pufs.
If no respone, or if attack is severe,
Inj.aminophyllin 1o ml + 25%glucose 10 ml i.v x very slowly or inj.adrenalin 0.5 ml subcut x children and young adult.
Oxygen by nasal catheter or mask.
If there is no relief in 10 minute,
Repeat inj.aminophyllin slow i.v or adrenalin s.c
Inj. Decadron 2 cc i.v or betamethasone or hydrocortisone.
Inj.gentamycin 80 mg i.v 8 hrly.
Inj.calmposed 1 stat & s o s
Oxygen by nasal catheter mask.
If there is relief still,
Refer to a physician.
Start aminophyllin drip-2 amps of aminophyllin in bottle of 5% dextrose x 20 drops/mins.
Inj.sodabicarb 50 ml x slow i.v injection.
Repeat inj.decadron as required.
After the attack subsides,start oral inhaled bronchodilator drug. If puls is very feeble-? Left ventricular failure. If liver is palpable, tender with leg edema-? C.C.F. If only lung signs-primary lung disease.

Inj. Morphin 15 mg I M or fortwin.
Inj.betnesol 2-4 amps i.v or decadron.
Lasix 1-2 amps i.v

Refer to hospital to rule out myocardial infract.

Propped up position.
Oxygen.
Inj.aminophyllin 10 ml + 25% dextrose 10ml x slow i.v or inj.deriphyllin 2 cc.i.m
Inj.lasix 2 amps i.v, (inj. i.m if B P is low).
Tab.lanoxin 3 stat, then 1 t d s x 3 days, then 1 od.
Tab.calmpose 5 mg 1 stat.
No I V fluids, no saline (no salt)
If lung disease,
Oxygen.
Inj.aminophyllin 10 ml + 25% dextrose 10ml x slow i.v or inj.deriphyllin 2 cc.i.m
Inj. Decadron 2 cc i.v or betamethasone or hydrocortisone.
Get x-ray chest as early as possible to decide further line of treatment.
SEVERE HEMETEMESIS OR HEMOPTYSIS
Step –1: quickly judge the source of bleeding
HemoptysisHemetemesis
If h/o cough, breathlessness expectoration.
If blood is coughed out & is mixed with sputum.
If blood is bright red, frothy with sputum.
Litmus paper = alkaline reaction.
If h/o epigastric ie, ulcer like pain. If h/o alcoholism & cirrhosis.
If blood is vomited out and is mixed with food particles.
If blood is coffee ground but it may be fresh red if variceal bleeding.
Litmus test = acid reaction.
Step-2 : start 18 no. I.V line
Start R-L or D N S, using 18 no.scalp vein needle and let the I.V run in very fast.
If lot of blood is lost, start hemacele/dextran (plasma expanders)
Make arrangements to shift the patient to a hospital with blood transfusion facilities.If hemetemisis,
Ryle’s tube and cold water stomach wash.
Inj.ranitidine 1-2 amps.I.VIf hemoptysis,
Inj.cefazolin 500 mg I.V 6 hrly.
x-ray chest and if hilar shadow, then bronchoscopy.
Anti-T B treatment, if tuberculosis.
Step-3 : measures to stop bleeding,
Inj.calcium gluconate 10 ml slow I.V stat.
Inj.dicynene 2 ml I.V stat. Or 4 hrly.(ethamsylate) or stryptohrome 2cc I M stat.(adrenochrome)
Inj.morphin 15 mg I M stat.
Inj.decadron 2 cc I V stat
SCORPION BITE (bichhu)
Patient present with very sever pain & sweating.
Inj. pethidine 100mg I.M/I.V stat. Or inj. fortiwin /norphin/morphin.
Inj. Local xylocain 2% : infiltrate at and around the site of bite (the site of bite is identified as pinpoint puncture spot, local sweating & edema ).
Tab. Prazopress 1 mg.stat. (prazocn = alpha bloker). Or ciplar 40 mg.stat. (propranolal ) In children, tab prazocin 1 mg x ½ -1/4 tab. Or tab.ciplor-10 mg. Tab. In most cases, where scorpion poison is mild, this treatment is sufficient. If there is systemic involvement,
Systemic involvement is indicated by – profuse sweating, ice cold extremities, priapism, hypersalivation, vomiting, hypertension, tachycardia & pulmonary edema.
Observe pulse rate, B P, & breathlessness, every 10 mins.
Tab. Prazopress 1 mg.stat ½ tab after 4 hrs, then ½ tab every 6 hrs till. Systemic symptoms & signs disappear, usually 24 to 48 hrs. or tab.ciplor 40 mg 1 stat.,1 after 4 hrs, then every 6 hrs.
Inj.fortwin 1cc I M/IV S O S if pain is severe.
I V. R L x if profuse sweating & dehydration.
If B P. > 150/100,
Tab. Depin 5 mg sublingual, to be repeated s o s after ½ -1 hrs (nifedipine).
If tachycardia >110/mins,
Inj.calmposed 2 cc I M or slow I.V
If breathlessness, fine fasal erepts i.e. pulmonary edema,
Propped up position.
Oxygen.
Inj. Lasix 2-4 amps I.V stat.
Inj. Efcorlin 100 mg I.V. stat
Inj. aminophyllin 10 ml + 25% dextrose 10ml x slow i.v or inj.deriphyllin 2 cc.i.m
In life threatening situations, with severe dyspnoea & frothing through mouth, give sodium nitroprusside drip till pulmonary edema is controlled.
Inj.pruside 50 mg in 5 ml x added to 500 ml D% x 15 drops/min x may be increased up to 40 drops /mins.
There is no specific antidote for scorpion poison. The magical effect of prazocin, which gives total protection against cardiac toxicity of scorpion bit was discovered by an Indian doctor, Dr. Bavaskar from mahad, raigad, district in maharashatra.
SNAKE BITE
First aid,
A reassurance is often the most important part of the panicked patient.
Tie a tourniquet above the site of bite on the limb, using a handkerchief or long piece of cloth, not too tight- just to occulude venous & lymphatic return.
Immobilize the limb with splint, because movements = more absorption.
If snake was poisonous, make the cruciate incision through the bite marks, and allow the blood to flow.Signs of poisoning.
Two puncture marks, 3/4-1 cm a part, should mark you suspicious of poisonous snakebite.
There are two type of poisons – paralytic type and hemolytic type
.
Paralytic type :
Stage- I : prosis.
Stage – II : difficulty in swallowing.
Stage – III: respiratory difficult & paralysis.
Hemolytic type:
Local swilling around the bite develops rapidly.
Blood collected in test tube does not clot.
If no signs of poison are seen,
Tab. Diazepam 1 stat (to relieve anxiety)
Tab. Disprin 1 stat (analgesic )
Inj. T.T. ½ cc I.M stat.
Reassure the patient.
Observe for 6 hrs, watch for the 4 signs of snake poison – local swelling, ptosis, dysphagia & respiratory difficulty. If signs of poison are seen,
Paralytic type:
Act immediate if ptosis or dysphagia is seen. If you do not have A.S.V, take the patient quickly to a hospital where A.S.V is available.remember that respiratory paralysis is imminent.
Inj.decadron 2 cc I.V stat
Inj. avil 1 amp. I.V stat
Inj. A.S.V. 1 to 4 vials, every 15 or 30 mins, till the signs start reversing (ASV-polyvalent). A.S.V. vial contains powder. Dissolve in 10 ml. distil water. Inj.½ cc I.V. as test dose. Wait for 2 mins. & Watch for urticaria. Then inject the full dose of a.s.v.
Inj. A.S.V.1 ml locally around the site of bite.

EUnani medicine concept believes that the basic component of the body are atoms, they interact with each other to form molecules these molecules, on further interaction form the four basic fluids (humors) of the body viz phlegm, blood, yellow bile and black bile.

These form the organs and in their appropriate proportion perform the physiology of the body with the help of organs.
When this proportion is disturbed it manifests in various pathologies.
These pathologies are predisposed by the six essential factors of life i.e. environment; foods and drinks; rest and body functions; emotions and psychological rest; sleep and wakefulness and retention and excretion.
When adhered to these factors in their appropriate measures promotes health and when disturbed in their appropriation predispose the body to humoral imbalances.
To aid the body of its ailments Unani medicine proposes a set of four practices starting with diet therapy followed by regimental therapy, drug therapy and surgery.
The great unani physicians and unani scholars define stress as a reaction to loss of an opportunity, substance or life; loss of a much desired substance or circumstances wherein; a person cannot avenge or curb an unfavorable situation.
However stress is also defined as an interaction between an individual and his environmental that he perceives as straining or exceeding his adaptive capacities and threatening his well being.
The element of perception indicates that human stress responses reflect differences in personality and physical strength which is also described as temperament of that individual.
Temperament is designed by the supervening humor of that individual.
Which in turn happens to be decided by two factors i.e. genes and the environment and in today’s world any long term medications.
Stress is of three types acute, episodic and chronic.
Acute stress is most common and refers to the pressures and demands of recent future.
It can be thrilling, tiring or exhausting.Frequent bouts of these acute stress are referred to as episodic stress.
It causes over arousal, short temper, irritability, anxiety and tension.
Chronic stress is from unrelenting demands and pressures for seemingly interminable periods of time.
It can prove fatal through suicide, violence, myocardial infarction, cerebrovascular accidents and perhaps even cancer.
Another type of chronic stress is post traumatic stress disorder wherein, the stressful episode keeps appearing in memory flashback and nightmares.
Causes
A certain degree of stress is a part of daily life and enriches it with aptitude, ambition and inertia.
These when extend over a period of time and extend a relentless demand on the coping ability of the individual then manifest the symptoms of stress related disorders.
Unani physicians consider dis advantageous interaction with the environment burn any one or all the four physiological humors into pathological melanchole.
This forms the basis of psychological and psychiatric disorders.
Stress is not a disease per se with the Unani physicians but a cause for chronic disorders like Koch’s, mania, fevers, etc.
Pituitary responds to stress by increasing the production of adrenocorticotrophic hormone (ACTH), which acts on adrenals to increase production of stress hormones viz cortical and adrenaline.
Symptoms
Stress can have both physical and psychological symptoms.
Long term stimulation of the adrenals by the pituitary under chronic stress causes physical symptoms like irritable bowel syndrome, myocardial infarction and chronic headaches.
Stress related emotional illness results from inadequate and inappropriate responses to major changes in ones life like completing one’s education, becoming a parent, losing a job or retirement.
Hence stress related problems can also be toned down as adjustment disorder.
High stress levels give a sense of burnout, loss of interest, inability to concentrate, etc.
Damage done to the body due to elevated levels of stress hormone is
Digestive System: Stomach ache or diarrheas are common symptoms of stress.
Cortisol delays the release of gastric secretions, stomach emptying and increases peristalsis.
It is also responsible for increased appetite and obesity.
Immune System: Cortisol levels are inversely proportional to immunity. The higher the cortical level the lower the immunity.
Occasionally it causes an overactive immune system predisposing to autoimmune disorders.
Nervous System: Increased adrenaline causes anxiety, a feeling of helplessness and impending doom.
Excessive cortical can cause sleep disorders, loss of appetite, loss of libido and depression.
Cardiovascular System: High cortisol causes tachycardia, hypertension, hyperlipidemia, myocardial infarction and cerebrovascular accidents.
Other Systems: Stress worsens may cause skin conditions such as psoriasis, eczema, urinary, acne. It can trigger an asthmatic attack.
Increased cortical levels may cause Cushing’s syndrome whereas; increased adrenaline predisposes to Addison’s disease.
Management (usoole ilaj)
The management of this malady involves a three fold strategy.
Distracting the mind through adaptive changes and distracting methods,
Excretion of pathological melanchole, and
Strengthening the heart and mind.
Adaptive changes must be introduced to make the patient comfortable and happy.
Amongst the adaptive changes are living in clean, well lit house, which has provisions for fresh breezes of air, wearing white or pastel shades, ensuring comfort and entrusting with responsibility.
The mind should be distracted to break the thought series through distracting methods which include:
Visiting hill stations, landscapes, parks, etc,
Viewing plays, movies, ect..
Hearing pleasant quranic recitation.
Being in the company of religious people,
Listening to religious sermons,
Listening to interesting stories, fairy tales,
Reading books,
Inculcating hobbies,
Participating in mentally challenging games like , quiz
Participating in competitions like extempore, debates, etc.
There are certain prohibitions too that should be adhered to for instance living in small congested houses, dressing in black dresses, exertion, anxiety, worry, starving, being lonely and insomnia as these induce a sense of mourning.
After the adaptive and distracting methods, comes the second and third line of treatment which involves excretion of the pathological melanchole and strengthening the heart and mind.
This is achieved by a combination of regimental therapy
1.(Ilaj bit tabdeer),
2.diet therapy (Ilaj bil ghiza) and
3.drug therapy (Ilaj bid dawa)
Sleep is an important and an integral part of the treatment of such disorders.
Oral use of sweet almond oil (roghan badaam shireen), lettuce (kahu) and compound formulations like (Sharbat Khashkhaash) are beneficial in inducing sleep due to their sedative and tranquilizing properties.
Diet therapy (ilaj bil giza)
Chappaties must be preferred to over rice.
Gourd and cucumber are recommended amongst the vegetables to patients suffering from chronic stress.
Both are documented diuretics which aid in the excretion of pathological melanchole.
Besides this Lagenaria Siceraria (kaddu) is also beneficial in delirium. Butter milk and curd should be a part of the prescribed diet.
Among the fruits, watermelon, pomegranate, sugarcane and grapes are helpful.
Citrullus Lanatus (tarbooz) and sugarcane are known for their diuretic properties.
Punica Granatum (anar) is a potent antioxidant.
Vitis Vinifera (angoor) is effective in degenerative nervous diseases.
Clinical trials have proven its efficacy in Alzheimer’s. It is also a reported nootropic diet with known adaptogenic and antioxidant effect
Constipation must be avoided and this can be attained by drinking warm milk sweetened with honey at bedtime or eating figs.
Alternately, 2 tsp of drinking sweet almond oil with warm water or milk too is beneficial.
Regimental therapy (ilaj bid tadbeer)
The regimens that are prescribed for chronic stress are:
Veinesection (Fasad)
saphenous veins should be dissected to drain the pathological melanchole circulating with the blood.
Sauna (Hammam)
cold water or lukewarm water can be used for this purpose.
Massage (Dalak)
Light massage with Viola Odorata (banafsha) oil or almond oil is advisable.
Shower (Natool)
Diluted rose water, Lactula Sativa (kahu) decoction and Viola Odorata (banafsha) are prescribed due to their exhilarant and sedative property.
Seitz bath (Abzan):
decoction of Lactula Sativa (kahu) and Viola Odorata (banafsha) are advised.
Cupping (Hijamath):
this should be done on the shoulders.
Drug therapy
Most common mufaradaat drugs that are beneficial in the treatment of chronic stress are below
1.Celastrus paniculatus is commonly known as (Malkangni), Kangani,
2.Matricaria recutita,( babuna) commonly known as chamomile
3.Melissa officinalis (lemon balm) badranjboya
4.Rauvolfia serpentina ( asroul) or Indian snakeroot orsarpagandha,
5.Withania somnifera, (asgand nagoori ) commonly known as asashwagandha
6.Emblica officinalis (Amla)
7.Lactuca scariola, (Tukhme Kahu)
8.Lavandula Stoechas ( Ustakhudoos)
Conclusion
Modern life is full of stressful situations which has turned the hitherto, normal feature of life into a causative factor for disease.
Hippocrates( buqrat) had described constipation to be the mother of all disorders, but in the present scenario stress can be conveniently and without any opposition be attributed to as the mother of all diseases.
The principles of treatment offered by the great Unani physicians still form the core of management of this which has been testified by modern medicine for its efficacy and usefulness.

He has narrted this in his own words :-
"I received a call from someone claiming that he was from my mobile Service provider
and
He asked me to shutdown my phone for 2 hours for 3G update to take place.
As I was rushing for a meeting,
I did not question,
but just shut down my cell phone.
After 45 minutes?
I felt very suspicious, Since the caller did not even introduce his name.
I quickly turned on my cell phone and saw several missed calls from my family members.
And, the others were from the number that had called me earlier.
I called my parents
and
I was shocked that they sounded very worried asking me whether...
I am safe.
My parents told me that they had received a call from someone claiming that they had me with them ,
and asking for money to let me free.
The call was so real and my parents even heard 'My Voice'
Crying out loud asking for help.
My father was at the bank waiting for next call to proceed for money transfer.
I told my parents that,
I am safe and asked them to lodge a police report.
Right after that I received another call from the guy asking me to shutdown my cell phone for another 1 hour which I refused to do and hung up.
They kept calling my cell phone until the battery had run down.
I, myself lodged a police report and I was informed by the officer that there were many such scams reported.
MOST of the cases reported that the victim had already transferred the money...!
And,
It is impossible to get back the money.
Be careful as this kind of scam might happen to any of us...!!!
Those guys are so professional and very convincing during calls.
If you are asked to
shut down your cell phone for updates by the service provider,
ASK AROUND...!
Your family or friends might receive the same call.
Be Safe and Stay Alert...!

The Bombay High Court has instructed Maharashtra Medical Council to ensure that allopathic doctors write their qualification as BMBS and not MBBS. This move comes after a homeopathic practitioner made a representation contending "Using the alphabet M as the first alphabet was giving people the wrong impression that MBBS is a postgraduate Masters degree like M.Sc, M.Com, M.A, M.B.A, etc. where as its is only a Bachelors degree. Due to this confusion, patients preferred MBBS doctors over BHMS doctors as they thought that MBBS doctors were higher qualified".
Reacting to the new development Dr. Janak D. Shah, President of General Practitioners Association, Mumbai, said that "We have no problem whether it is MBBS or BMBS. But it is wrong to believe that patients prefer MBBS over BAMS or BHMS doctors because the name of our degree starts with the letter M. Patients very well know that MBBS doctors are far more talented and efficient at using allopathic medicines, which is why they prefer us over anybody else". President of Maharashtra Medical Council Dr. Kishor Taori said that the court has given 180 days of time period to implement this change. "We would be putting up a public notice on our website and also ask IMA to propagate this development through it's vast network" he added. When asked about the penalty doctors could face for not complying with the directive, Dr. Taori said that it would be decided at the upcoming meeting of the Executive Council of the MMC.

Slight drizzling and Roads were under repairing in many area and I need to cover 35kms in 25 minutes
By almighty Allah's Grace I crossed the check post exactly at 8.57pm
Covered 35kms in 22 minutes
I don't know whether it's because of Toyota Fortuner or because of it is my driving skill
But definitely I feel I'm still young & energetic @ 55 years of age.

He would chew it until it was soft and then rub it onto the palate of the new born baby. This is called "Tahneek". Specific details are on Wikipedia, recent Medical journals of Paediatrics and Neonatology besides the www.
As late as 2013, scientific research in Medicine published on LANCET (leading medical journal) was covered by BBC News, reports that "experts" have said - "A dose of sugar given as a gel rubbed into the inside of the cheek is a cheap and effective way to protect premature babies against brain damage"
This is why Muslims follow the Sunnah, of the messenger without questioning it. 😊
Science is only now discovering a solution. Among Mohammedan tradition this is a practice, that was introduced 1436 years ago. Islam was and still is the forefront of development.

Recently, one person was admitted to a Wellknown nursing home at pune, due to severe chest pain. He had an earlier attack in 2012 and was under treatment. The doctors now suggested Angiography.
Upon undergoing Angiography at multi speciality Hospital Doctors diagnosed multiple blockages for which Angioplasty was ruled out and instead, suggested 'Bypass Surgery'.
That evening, he was brought home as doctor suggested his heart being very weak, bypass could be performed only after 10 - 15 days.
Meanwhile, after discussing the matter with relatives and close friends, fresh information came from a family friend.
A new treatment known as-"Chelox therapy, Laser therapy & Chelation Therapy" has been introduced into the Indian medical theatre.
With this therapy, a patient who has to undergo by-pass need not do so.
Instead, the patient is given about 30 bottles of IV fluids in which certain medicament are injected. The medicine cleans the system and removes all blockages from the heart and the arteries. The number of bottles given may increase depending upon the age-factor and health of the patient.
Cost per bottle may be around Rs.1300/- .
Currently, only a few doctors in India specialise in this field
One of them is DR.Vikrant Laate. In pune.
He has a list of patients who had to undergo by-pass from major hospitals; but, instead after undergoing the new treatment, they are absolutely fine and are leading a normal life. Without or minimum medicine.

Methods: 24 healthy volunteers were enrolled for this study. Venous blood samples were drawn and then wet cupping therapy was performed and wet cupping blood samples were taken. All samples were analyzed for measuring the levels of heavy metals by ICP-MS.
Results: We found that all selected heavy metals levels were significantly higher in wet cupping blood than venous blood.
Conclusion: We may conclude that one of the mechanisms of action of wet cupping therapy may be removing heavy metals from the body.

To know if you're drinking enough water, it is often said, just check if you are feeling thirsty. If you aren't, your fluid intake is likely to be just about right. But downing glasses of water along with your meals may not be the best time to quench your thirst.
Macrobiotic counsellor Shonali Sabherwal explains why you should not drink water during your meal. "Most Indians have water along with their meals. The usual theory is washing down the food while eating. People have no idea how wrong this practice is and how difficult this can be for their digestion. For those suffering with digestion problems, the ramifications are manifold. Our stomachs have a knack of knowing when you will eat and starts releasing digestive juices immediately. If you start drinking water at the same time, what you are actually doing is diluting the digestive juices being released to digest your food, thereby hindering them from breaking down food."
Research shows that sipping a little water during meals isn't a cause for concern but drinking a glass or two may interfere with digestion. It is best to drink fluids before and two hours after meals as this helps in absorption of nutrients, researchers have found.
Elaborating on what exactly happens inside you when you glug water during meals, Sabherwal says that it gets absorbed by the intestinal walls of the stomach. "This absorption continues till it becomes concentrated enough for the digestive juices to begin digesting your food. However, due to it getting mixed with water, this concentrated substance is now thicker than the food contents present in your stomach. So less gastric juice will be secreted to digest your food. The result is, undigested food leaks into your system as it gets absorbed through the stomach walls. This will also lead to acid reflux and heart burn." Drinking water with meals can also cause a surge in your insulin levels, almost like the way high glycemic food would affect you, Sabherwal cautions. "The more insulin is released in to your blood stream, the higher the chances of you storing fat in your body."
To avoid watering down your meals, Sabherwal suggests a few don'ts. "Make sure your food is not too salty as that would aggravate your thirst and trigger your need to down more water. Besides, eating in a hurry will cause you to gulp your food down. Most likely, you would feel the need to wash it down with water while you are at it. Instead, chew, chew and then chew some more. We secrete a lot of digestive juices (enzymes) while chewing, which makes our stomach's job easier " .

👎 Do not live in the hospital or its very close proximity so that patients and relatives want to meet you at odd hours (for you), but right time for them after having finished all their work.

To know if you're drinking enough water, it is often said, just check if you are feeling thirsty. If you aren't, your fluid intake is likely to be just about right. But downing glasses of water along with your meals may not be the best time to quench your thirst.
Macrobiotic counsellor Shonali Sabherwal explains why you should not drink water during your meal. "Most Indians have water along with their meals. The usual theory is washing down the food while eating. People have no idea how wrong this practice is and how difficult this can be for their digestion. For those suffering with digestion problems, the ramifications are manifold. Our stomachs have a knack of knowing when you will eat and starts releasing digestive juices immediately. If you start drinking water at the same time, what you are actually doing is diluting the digestive juices being released to digest your food, thereby hindering them from breaking down food."
Research shows that sipping a little water during meals isn't a cause for concern but drinking a glass or two may interfere with digestion. It is best to drink fluids before and two hours after meals as this helps in absorption of nutrients, researchers have found.
Elaborating on what exactly happens inside you when you glug water during meals, Sabherwal says that it gets absorbed by the intestinal walls of the stomach. "This absorption continues till it becomes concentrated enough for the digestive juices to begin digesting your food. However, due to it getting mixed with water, this concentrated substance is now thicker than the food contents present in your stomach. So less gastric juice will be secreted to digest your food. The result is, undigested food leaks into your system as it gets absorbed through the stomach walls. This will also lead to acid reflux and heart burn." Drinking water with meals can also cause a surge in your insulin levels, almost like the way high glycemic food would affect you, Sabherwal cautions. "The more insulin is released in to your blood stream, the higher the chances of you storing fat in your body."
To avoid watering down your meals, Sabherwal suggests a few don'ts. "Make sure your food is not too salty as that would aggravate your thirst and trigger your need to down more water. Besides, eating in a hurry will cause you to gulp your food down. Most likely, you would feel the need to wash it down with water while you are at it. Instead, chew, chew and then chew some more. We secrete a lot of digestive juices (enzymes) while chewing, which makes our stomach's job easier " .
